Where is the Clinic Located? 

 

  •       The Clinic is located at 202 South Commerce Cove.
  •       Take Hwy. 7 South towards Water Valley. 
  •       We are 2.5 miles past the intersection of Belk Blvd. and Hwy. 7. 
  •       Turn left onto South Commerce Cove and follow it around to the back.
  •       The sign in front says “OMMC”

  

 

Who can use the Clinic?

 

  •    Residents of Lafayette and Yalobusha Counties who:
  •       Are working 27 hours per week
  •       Have no health insurance, Medicaid, Medicare, VA benefits or Social Security income
  •       Have not applied for or do not receive disability benefits
  •       Meet the income guidelines

 

 

Who provides care at Oxford Medical Ministries Clinic?

 

  •       Licensed doctors, nurses, pharmacist, medical technologists and other health care professionals who volunteer without pay
  •       Office staff and lay volunteers

  

 

What services are provided at the Clinic?

 

  •         Primary care for adults with chronic illness.
  •         Limited laboratory testing ordered by OMMC doctors
  •         Prescription medicines (no controlled drugs)
  •         Health education counseling, basic x-ray

 

 

When is the Clinic Open?

 

 

  •        Monday – Thursday 9:00 a.m. – 4:00 p.m.
  •        Patients are seen by physicians Tuesday night; other days are used for screening, referrals, pharmacy services, monitoring and patient education.

 

 

What do I need to qualify for services?

 

  •       Call for an appointment when you have the following documents.
  •       Proof that you live in Lafayette or Yalobusha County, Mississippi ( ex. Utility Bill)
  •       Two most recent pay stubs from your place of employment
  •       Photo ID
  •       Your Social Security card and cards for each member of your household
  •       Most recent Federal Tax Return & W-2’s

  •       Do not receive care for chronic diseases
  •       Lack regular access to medications
  •       Receive fewer diagnostic and treatment services
